I get this occasionally, I've found that keeping my knees warm helps to lessen the problem loads. I usually run knee warmers when XC gaying it up, that keeps my joints nice and warm. I wear pads for everything else, they keep my knees toasty I'm also going to use it as a valid excuse to move to a warmer country

if the clicking/pain can be traced to the lump on the outside of your knee it could be your ITB, google ITB stretching to cure it (and folow the advice lol) That and pester the NHS if you feel your getting nowhere get a second oppinion, or a third. Im on; 2 doctors 3 month wait 1 physio (useless) 1 doctor (referal to other physio) 4 weeks wait 1 extended scope physio 1 week 1 physio about 6 sessions to miss diagnose the problem and set me up with some very effective treatments. In their defense they nearly got it right and it dod help by relieveing th estrss on the bad bit of cartilage. 1 more doctor (after several months tooing and froing and waiting) 1 consultant (surgeon) after a few months getting the opperation in exactly a month to hopefully fix it properly

Should really rest up if it starts to hurt, never try and push past the pain, as you're only going to damage it... impact seems to be a big cause, the majority of people in for knee and hip replacements that I met when in hospital were there through running lots, in the army etc. worst was a guy getting a double hip and a single knee replacement, he was a paratrooper for his whole career, put it down to landing from jumps with full kit and that...
I take Glucosamie Chonroitin everyday, meant to help maintain the cartilidge, not proven to totally work but it's not been found to be harmful in any way....
Main one is to keep the saddle up higher if you're doing a lot of pedalling, i don't mean DH as you will be standing up 99% of the time, but put the seat up otherwise, takes a lot of stress off the knee.

I was diagnosed as having altered patello femoral tracking (or something along those lines anyway :p) a while back.
Basically, as a result of lots of cycling, the muscles on the outer side of my leg developed more than those on the inside, causing my kneecaps to be pulled slightly out of position. This was more prominent on my left knee - presumably because my right leg is slightly stronger due to being my favoured foot in football.
Apparently, this is very common among cyclists.
Anyway, I've sorted out the muscle imbalance now, by doing exercises that target the weaker parts of my legs. The clicking still remains though (it's more like crunching, actually :/). My physio said that it should go away with time, as long as the muscles don't becoming imbalanced again, but I'm not convinced. I'm reluctant to say they're getting worse, but they really aren't getting any better. :'(
